You should use a vector image creator (eg. Inkscape) to draw the necessary shapes in the right dimensions.
A skin template can be found here: download/file.php?id=17403
Unfortuately we dont have a full tutorial for vectors, just one tutorial using GIMP (see viewtopic.php?f=16&t=6979).
So a quick step-to-step (might be wrong ?):
1) Open 'teeworlds_skins.png' in Inkscape
2) Draw your skin in a new layer (Take a look at existing skins to get an idea what it should look like)
3) Export as RGBA-png
4) Dilate / Process viewtopic.php?f=16&t=4940 viewtopic.php?f=16&t=2415
3) Place in %appdata%/skins and try it ingame
